Patient Rights and Responsibilities:

We encourage Patients to participate in their health care decisions. We therefore promote the following:

Patient Rights:

You have the right to privacy and confidentiality regarding your office visits and records.You have the right to adequate education and counseling regarding your medical condition.

You have the right to have all procedures, risks, benefits and alternatives explained and your questions answered in lay language.

You have the right to have the effectiveness and possible side effects of medications explained to you.

You have the right to see the results of tests and to have the meaning of the test results explained.

You have the right to participate in decisions made regarding treatments, medications, procedures and surgery.

You have the right to refuse treatment to the extent permitted by law, and the right to receive information on alternatives and the possible consequences of refusing treatment. 

You have the right to view your medical records, have them explained to you and request copies.

You have the right to an explanation of all fees.

You have the right to decide whether or not to participate in clinical research studies.

Patient Responsibilities:

You have the responsibility to give honest, accurate and complete medical history information.

You have the responsibility to make sure you understand what the doctor is saying and if you do not understand, to ask questions for clarification.

You have the responsibility to follow your doctor’s medical advice and instructions. 

You have the responsibility to report any significant changes in your health to your doctor. 

You have the responsibility to respect office policies and procedures.

You have the responsibility to keep scheduled appointments or cancel in advance. 

You have the responsibility to provide accurate insurance  billing information and be responsible for payment of personal balances in a timely manner.

You have the responsibility to present yourself to this office in a manner that is not offensive to others,      including proper attire and cleanliness.

You have the responsibility when in the office or on the telephone, to conduct yourself in a civil manner and not be abusive to the Doctor or Staff.
