Chronic Kidney Disease Program 

We are listing the most commonly asked questions with answers so that you may understand our CKD-Program.  We are happy to discuss all of your questions.

QUESTION: What is Chronic Kidney Disease (CKD)?
ANSWER: CKD is the permanent loss of a certain portion of kidney function.

QUESTION: What causes CKD?
ANSWER: CKD may be the result of family history or a physical injury or disease that damages the kidneys, such as diabetes or high blood pressure.  When the kidneys are damaged, they do not remove wastes and extra fluid from the blood as well as they should.

QUESTION: How will I know if I have CKD?
ANSWER: Your nephrologist (kidney doctor) will order and evaluate special blood, urine and other medical tests to determine if you have CKD and will discuss the best plan for your individual needs with you.  Your nephrologist will also communicate with your Primary Care Physician (PCP) about your treatment plan.

QUESTION: What is the Kidney Care Team?
ANSWER: The Kidney Care Team consists of the Nephrologist, Nurse Practitioner (NP), Renal Dietitian (RD) and support staff.

QUESTION: How does the CKD-P work?
ANSWER: Your Nephrologist and/or NP will be available for your office visits.  You will see the RD in a private session and/or group sessions based on your need.

QUESTION: Will I still continue being followed by my Primary Care Physician (PCP)?
ANSWER: Yes.  Your PCP will continue your care for all of your other health care needs, including your annual exams, flu shots, etc.

QUESTION:  What will the CKD-P cost me?
ANSWER:  Our Billing Department will contact  your insurance company to verify your eligibility for  nutritional counseling.  If you have questions related to billing and your insurance status, please contact our Billing Office Supervisor at 559-228-6600.

QUESTION: If my question is not addressed on this fact sheet, whom do I contact?
ANSWER: You are welcome to communicate as needed by calling our office at 559-228-6600.  Our office hours are Monday through Friday from 8:30 AM to 5:00 PM.
